Upstream SGI says it is caused by a DM lockup, not XFS code. http://oss.sgi.com/bugzilla/show_bug.cgi?id=860 Opened Bug 568319 - DM lockup causes filesystem failure Mounting nobarrier avoids the issue in 11.2 - per the suggestion in the SGI bugzilla.
