It was most probably just after or during first login into KDE, just after installation. I am to lazy to type it so there will be a screenshot attached. Error didn't get into any logs but was just on vt0.

This isn't ext4, it's preload. In userspace.

Stephan, is this a bug in preload or preload detecting a file system problem?
