At least in my setup (openSUSE 11.2 and KDE4, both official and 4.4RC), the "help" menu in KDE applications will not work as long as the package "susehelp" is not installed.
If "susehelp" is an integral part of SuSE's version of the KDE help system, it should be pulled in automatically.
